Download the accident claim form.
Step 2:
Complete the form.
Fill out the form entirely, making sure it is signed by the Team Representative or League Executive before submission.Â
*Incomplete forms may cause delays in processing your claim.Â
Step 3:
Upload and submit the completed document using the form below.
Step 4:
Collect and Submit Receipts.
Gather applicable receipts to your claim and submit these receipts to the CARHA Hockey office. Claim file will remain open for 52 weeks from the date of injury for approved claim forms
Expect a 2–3-week turnaround for claims to be processed.
*Important: All forms and receipts are to be submitted to CARHA Hockey with the Sport Accident Claim Form or it cannot be processed. You must see a physician or dentist within 30 days of injury and submit your claim form to CARHA Hockey within 90 days of injury in order for the claim to be considered
